


Each year, thousands of Canadians with diabetes develop advanced kidney disease, often without early warning. Together, we can change that.
CHEP+ and Diabetes Canada are launching the Diabetic Nephropathy Prevention Program (DNPP), a national peer-to-peer initiative helping clinicians improve early detection and prevent diabetic kidney disease.
As a Peer Clinical Educator, you will:

Promote early ACR screening and timely intervention

Empower colleagues to improve patient outcomes in your community
Join our Train-the-Trainer program to receive full support from CHEP+.
After training, you’ll lead 2–3 small-group learning sessions with peers.
About This Program
About the Program
The Train the Trainer Program equips clinicians and educators with the tools, knowledge, and resources to deliver high-quality education within their regions.
Through this initiative, participants will:

Receive accredited training on CKD screening and management in diabetes, aligned with the new 2025 CKD Diabetes Canada recommendations.

Access a comprehensive suite of learning materials, including case studies, slide decks, and facilitator guides.

Participate in interactive workshops and webinars led by national experts.

Gain the ability to deliver local training sessions to primary care teams and community health providers.

Apply the new 2025 CKD Diabetes Canada recommendations in their practice for improved patient care

Gain access to CHEP+ accredited online programs and conferences and receive modest honorarium

Become a CHEP+ certified facilitator able to deliver educational programs
Target Audience
This activity has been designed to meet the educational needs of healthcare professionals (doctors, nurses, pharmacists, and other healthcare educators/promoters) working with people with or at risk for cardiovascular and cerebrovascular disease.
